How does laser blended vision treat presbyopia?

Laser blended vision may offer an alternative to glasses for people with presbyopia, an age-related decline in near vision that commonly develops after 40. The procedure corrects one eye primarily for distance and the other for near vision. The brain then combines the different visual inputs to provide functional vision across multiple distances.

However, not every patient adapts well to this approach. Identifying suitable candidates before surgery remains important for ophthalmologists and refractive surgery specialists.

New research presented at the 44th Congress of the European Society of Cataract and Refractive Surgeons (ESCRS) suggests that a straightforward clinical assessment may help predict whether patients can tolerate blended vision.

Can a simple eye test predict suitability for laser blended vision?

Researchers led by consultant ophthalmologist Brendan Cummings of the Wellington Eye Clinic in Dublin used a phoropter, a standard device used during routine eye examinations, to assess patients before surgery.

First, clinicians corrected each patient’s vision for distance and asked them to consider this their 100% visual benchmark. They then deliberately defocused one eye at a time and asked patients to compare their perceived vision with the benchmark.

Patients who reported at least 80% of their baseline distance vision while one eye remained defocused appeared more likely to tolerate the different visual inputs required for blended vision. The assessment also helped clinicians identify the dominant eye, which can guide treatment planning by assigning distance correction to the dominant eye and near correction to the other.

What did the study find about patient selection?

The study included 1,341 patients with different refractive profiles, including myopia, hyperopia, and no significant refractive error.

Preoperative testing identified a small group of patients who were considered unsuitable for the procedure. Overall, 96.9% of patients successfully adapted to laser blended vision after undergoing the screening process and surgery.

Cummings and colleagues concluded that perceived distance vision and ocular dominance provide useful information about how a patient’s visual system may process different inputs.

For ophthalmologists, the findings offer a practical approach to patient selection for laser blended vision without requiring specialized sensory-dominance technology. Dr Joaquín Fernández, who was not involved in the study, noted that clinically accessible selection criteria can support treatment planning and help assess the cost-effectiveness of newer technologies.

For HCPs involved in refractive surgery, a simple preoperative assessment could therefore provide valuable insight into whether a patient’s visual system is likely to adapt to blended vision.
