What Does the Evidence Say About Nicotine E-Cigarettes for Smoking Cessation?
Nicotine e-cigarettes may help more smokers quit than several established cessation approaches, according to an updated Cochrane living systematic review. The analysis included 80 randomized controlled trials involving 29,861 participants, including nine trials added in the latest update.
The review found high-certainty evidence that e-cigarettes resulted in higher smoking cessation rates than nicotine replacement therapy, such as nicotine patches and gum. For every 100 people using these e-cigarettes, approximately four more people quit smoking compared with those using NRT.
Nicotine e-cigarettes also performed better than non-nicotine ones, producing approximately two additional quitters per 100 people. Quit rates were also higher when e-cigarettes were provided alongside no support or behavioral interventions, although researchers rated this evidence with less certainty because of methodological concerns.
Are Nicotine E-Cigarettes Safe for Smoking Cessation?
Researchers reported no evidence of increased serious harm associated with e-cigarette use in the trials reviewed. However, the authors emphasized that longer-term studies are necessary to better understand potential health effects.
The findings specifically concern regulated nicotine e-cigarettes. Illicit or unregulated products can present different risks, including exposure to harmful chemicals and device-related hazards. E-cigarettes also expose users to nicotine and other substances, making them unsuitable for people who do not already smoke.
Health professionals should therefore distinguish between switching from combustible cigarettes to regulated e-cigarettes and initiating vaping among nonsmokers. Youth nicotine exposure remains a major concern because e-cigarette use can contribute to nicotine dependence and subsequent tobacco use.

Smoking cessation remains the primary goal, and clinicians can help patients select an appropriate cessation strategy based on their smoking history, previous treatment attempts, preferences, and clinical circumstances. Behavioral counseling and established cessation therapies remain important components of comprehensive support.
