What Is Cardiovascular Frailty and How Is It Linked to Heart Health?

Cardiovascular frailty may represent an important connection between age-related physiological decline and cardiovascular health. Frailty reduces physiological reserves and leaves older adults more vulnerable to health deterioration after relatively minor stressors.

Traditional tools, including the Rockwood Clinical Frailty Scale and Fried phenotype, primarily assess cumulative deficits or physical frailty. However, researchers note that these approaches do not fully capture vascular, cardiovascular, cognitive, and psychosocial factors.

Evidence indicates a bidirectional relationship between frailty and cardiovascular disease. Pre-frailty and frailty have been associated with approximately 70% higher cardiovascular disease risk, while frailty has been linked to nearly three times the risk of cardiovascular death.

Researchers suggest that oxidative stress, chronic inflammation, cellular senescence, endocrine changes, DNA damage, and epigenetic alterations could contribute to this relationship.

How Do Vascular Aging and Blood Pressure Affect Frailty?

Vascular aging causes structural and functional changes in blood vessels and can contribute to arterial stiffness. Studies have associated arterial stiffness with frailty, disability, and the development of isolated systolic hypertension in older adults.

Blood pressure regulation also becomes more complex with aging and frailty. Older adults may experience hypertension, isolated systolic hypertension, orthostatic hypotension, or white coat hypertension. Blood pressure variability, autonomic dysfunction, endothelial impairment, and vascular stiffness can contribute to these conditions.

Diabetes, atrial fibrillation, chronic kidney disease, stroke, and heart failure may further complicate cardiovascular risk in frail older adults.

Evidence from the HYVET trial analysis found that frailty did not appear to alter the benefits of antihypertensive treatment among adults older than 80 years. However, observational studies have linked lower blood pressure with excess mortality in frail older adults. Current hypertension guidance therefore emphasizes individualized clinical judgment and assessment of treatment risks and benefits in older adults with frailty and multimorbidity.

Could Oxi-Inflammaging Explain the Frailty-Heart Connection?

Oxi-inflammaging, the interaction between oxidative stress and inflammation during aging, may represent another biological pathway connecting cardiovascular aging with frailty.

Older adults with frailty often show higher oxidative stress and inflammatory biomarkers and lower antioxidant levels. Poor nutrition, physical inactivity, genetic susceptibility, and age-related conditions may contribute to this process.

Similar mechanisms also play a role in cardiovascular disease, suggesting that shared biological and lifestyle factors could influence both cardiovascular frailty and cardiovascular outcomes. However, much of the existing evidence remains cross-sectional, so researchers cannot establish causation.

The review proposes developing a cardiovascular frailty phenotype and index incorporating vascular aging, blood pressure regulation, oxi-inflammaging biomarkers, and clinical frailty measures. Such a tool could eventually help clinicians identify cardiovascular vulnerability earlier and tailor management. Further longitudinal research and validation remain necessary before clinical implementation.
